Put in the email address and password that you used to enroll your card.

Put in our group number (83164) or GREATER OAKLAND MOTHERS OF MULTIPLES

Choose the correct organization.

Click Enroll.
It is important to note that once you enroll you need to re-enroll every year after April 1st.
